Library file picture dated 6/10/99 of rail signal 109 in west London, which had been passed at danger seven times in the five years before the October 1999 Paddington rail crash. The rail industry was expected to face fierce criticism, Tuesday June 19 2001, over safety in the official public inquiry report into the disaster which claimed 31 lives. Railtrack, Thames Trains and successive governments were all expected to be blamed in Lord Cullen's report into the accident at Ladbroke Grove, near Paddington station in west London.
